No, you do not require a referral to make an appointment.

+ How do I make an appointment?

Please contact me using one of the options on the Contact page. The easiest way is to either fill out the online form or to email moniquejcormack@gmail.com.

+ What should I bring to my appointment?

If you have any relevant test or examination results, like from previous blood or urine tests, please bring them along. If you’re taking any supplements, then it would be great if you could please bring (1) a photo of the supplements so I can see all the information or (2) a detailed list noting the brand name, supplement name, dose and details of how much/how often you take.

+ What happens if I need to change or cancel my appointment?

A 24-hour cancellation policy applies to all bookings. Any cancellations within the 24 hours prior to the appointment time will incur a cancellation fee of 50% of the original booking value.

+ When do I pay for my appointment?

Online appointments require a 50% deposit to be paid before the scheduled appointment. Appointments at the newLife Centre for Women are paid for at the time of the appointment in the clinic.
